Use this optional checklist when the appointment includes vaccination. It helps organise questions and records; ticking every box does not mean a baby is medically ready for vaccination. The clinic decides what is appropriate for your child.
Keep the paper Child Health Booklet with your appointment supplies. HealthHub asks parents to bring it so staff can record vaccinations and assessment findings. Let the clinic know about records from another provider, a missed appointment or a previous vaccine reaction. Ask the clinic how to proceed if baby is unwell; do not use this checklist to decide whether to delay or go ahead.
